Surcharge Reporting Form - Effective August 1st, 2026

Please be advised that effective August 1, 2026, the Black Car Fund surcharge rate for passengers decreased from 2.5% to 2.25%. Below is a notice that was sent to Member Bases informing them of this decrease, and above is an updated surcharge form to be used for all Covered Services performed on or after August 1st, 2026

Electronic Surcharge Remittance

Member Bases can elect to remit their monthly surcharge to The BCF electronically using the appropriate Credit Card Surcharge Reporting Form below.

Please select the form that corresponds with the surcharge reporting period:

February 2025 through July 2026: Use this form: Feb 2025 - July 2026

August 2026 and to date: Use this form: August 2026 - Present

Payments for reporting periods prior to February 2025 must be submitted using the following Surcharge Reporting Form
