If you are interested in reporting a work-related accident to The Fund, please do so electronically through The Black Car Fund App, or our website. You can also call our offices during business hours (Monday to Friday from 9 AM to 5 PM) to report your accident to our Claims Team. If you have an existing claim, please communicate with your assigned adjuster via email or phone. Medical Bill Submission

Medical bills (including Pharmacy bills) MUST be submitted, including all required information, to:

Preferred Method: Electronically via a clearinghouse. Our Payor ID is LT685. (If you submit a paper CMS-1500 bill on or after August 1, 2025, The Black Car Fund will automatically deny your bill and not send you payment. Instead, you will receive an Explanation of Benefits that references CARC 16/RARC N34 codes, which indicates your bill was not submitted electronically. In order to be paid, you MUST resubmit the bill to us electronically through an approved electronic submission partner within 120 days from the day medical care was rendered. Failure to submit the bill in a timely manner electronically will result in a denial for untimely submission.)
