Important Notice Regarding Medical Bill Submissions
Effective August 1, 2025, all medical and pharmacy bills must be submitted electronically, using the required formatting and details, through a recognized clearinghouse.

Please note: Beginning August 1, 2025, paper submissions using the CMS-1500 form will no longer be accepted for payment. If a paper bill is received on or after this date, The Black Car Fund will issue an Explanation of Benefits (EOB) or Explanation of Reimbursement (EOR) citing CARC 16 / RARC N34 codes, which indicate the bill was not submitted through the required electronic method.
To receive payment, the provider must resubmit the claim electronically through an approved clearinghouse within 120 days from the date of service. Failure to do so will result in claim denial due to untimely submission.
